Preparation of Records for UAE

 Five things you need to do before a UAE procedure

1. Insurance Pre-certification
 

It is necessary to obtain pre-certification for UAE (approval by your insurer to have the procedure done.) from your insurance company before proceeding.

Call Our Program Coordinator, at (888) 666-2002 to initiate the processing of your insurance claim, and to help you determine what your insurer will cover. If you have no insurance, or your policy does not cover UAE, Maria can also speak to you about setting up a payment plan.

 

2. Prior Medical Records and Referrals
All Records (a) and Referrals (b) noted below will be available from your Gynecologist's office.
 

(a) Copies of Records
These include
(but are not limited to):

  1. Copy of your most recent pelvic examination
  2. Most recent Pap Smear History
  3. Pathology Reports from prior D&Cs or other pelvic surgery which your gynecologist believes are relevant
  4. Operative reports from prior gynecologic procedures which your gynecologist believes are relevant
  5. A copy of any Previous Pelvic Ultrasound reports; previous MRI reports
  6. Any other records that your Gynecologist deems necessary

If you have significant medical problems, a report from your Internist with recommendations is also appropriate.


(b) Referrals

  1. MRI - if you have not had a recent MRI, you may need a referral for this.
  2. GYN "Second Opinion" - if your gynecologist is not a member of Northern Westchester Hospital staff, you will need a referral from your own gynecologist for this consultation..
 

 

3. Gyn and Medical History Form

4. Make an appointment for a consultation
with Dr. Forcade.
 

At that visit Dr. Forcade will review your history, discuss the procedure, and answer any questions specific to you. The consultation may include an appointment for an MRI if you have not had this examination recently.

5. Gynecologic Coverage while at NWH
 

You will need a Gynecologist for a "second opinion" and as a backup surgeon from our hospital staff.

If your gynecologist is on staff at our hospital, he/she will satisfy the requirement. Otherwise please select one of the following physicians and make an appointment for this "second opinion" at their office as soon as you learn the date of your procedure. (This referral should be made by your gynecologist.)

The necessity to have a Gynecologist "On-Call"

For any questions related strictly to gynecologic matters, or should there be the need for surgical intervention - a gynecologist becomes an important part of the team. In the event a patient's own gynecologist is not on staff - we must involve one of our physicians. Frankly we have not had the need for any surgical intervention in over 250 cases thus far. However, safety at all times is a basic requirement during any procedure.
